Dr Watson has also became smarter

Dr Watson was making his way back to Baker Street when he bumped into an old collegue whome he hadn't seen or heard of for twenty years. They had been in a medical school together and had graduated at the same time.
" Where are you Now ?" asked Watson.
" I am the head surgeon at London's Hammersmith Hospital, " came the reply
" And this little girl, " said Watson , "
 is she ours?"
" Yes , I ve been married for eight year now."
The litttle girl tugged on watson's jacket. "My name"s Mary, " she said.
" Fancy that, " replied Watson patting her on the head.
" The same name as your mother.

How did Watson Know that ?

  1. It's entirely possible that Watson knew the mother in question when he and his colleague were in school together.
    OR
The mother is the colleague. You might be measuring our ability to subdue our natural impulses and guesses that have a good chance of being wrong. If so, I commend you. If not, give it a try sometime.

    1. Women in Dr Watson's time were not allowed to study, unless you are talking about the modern version. In that case, the mother is the collegue. Otherwise the man is the collegue.
In that case, he must have noticed a parcel/note/gift/anything addressable, adressed to his wife. You dont mention it, so...
Also, his collegue would have had some feelings for her in college and told Watson. And the young girl had her mother's face-cut.
